Can't find your card? Getting a new C-card has never been easier!
If you need a replacement card, there are two simple ways you can get one:
1. You can contact
your
SSI Diver Training Facility to order another card. This is the least expensive option for you, and is recommended if it is not an emergency situation.
2.
You can order a card directly through SSI. Just print out the
Replacement Card Form*, fill it out, and mail
or
fax
it to SSI
